NextGen Ministry Application


The intent of this application is to reduce the risk of child abuse in any form throughout our church environment. It is designed to protect children and youth, as well as to uphold the personal dignity of all who would be involved in serving the ministries associate to children and youth. All information on this application will remain confidential within the legal requirements of the church.

Criminal History: If you prefer, you may discuss your answers to any of the following questions pertaining to Criminal History with a pastoral staff member rather than answering it on this application. Answering yes or leaving the question unanswered will not automatically disqualify an applicant for children’s/youth ministry. As representatives for the International Church of the Foursquare Gospel, we are legally required to ask these questions.

Applicant’s Release Statement, Declaration of Moral Integrity & Digital Acceptance

“The information contained in this application is correct to the best of my knowledge. I authorize and release all references or churches listed on this application to provide any information that would be helpful in determining my fitness for this ministry regarding my character or abilities and release those references or churches from all liability for furnishing evaluations to the pastoral staff of the Foursquare Church, dba theBridge, providing they do so in good faith and without malice. I understand that these evaluations will be confidential, to be seen only by the pastoral staff.”

“Should my application be accepted, I agree to be bound by the Bylaws of the International Church of the Foursquare Gospel, the policies of theBridge and the guidelines of the ministry assignment. I will refrain from unscriptural conduct as it relates to my performance of this ministry. I will conduct myself in Godly manner, realizing that my personal walk with God is of utmost importance and the example I set will influence the children or teens to which I minister. I will commit myself to the ministry for a period of one year, at the end of which my continued service will be determined based upon my desire, my performance and the pastoral staff's approval. I will also commit myself to the direction and vision as it seen by my ministry leader.”

As an applicant for a volunteer position with unsupervised access to children/youth at theBridge and its ministries, I recognize, understand and agree to live by the moral standards of the church.

I further declare that with regard to my personal moral character and conduct as of this date, I am not engaged in inappropriate conduct, nor do I have inclinations toward such conduct.

Inappropriate conduct includes, but is not limited to, such behaviors as the following: adult heterosexual activity outside of a marriage commitment; homosexual or lesbian inclinations and actions of any kind; pornography; sexual abuse or improprieties towards minors as defined by Scripture or state law.

I, the applicant (also known as “consumer”), concurrently authorize theBridge through its independent contractor, LexisNexis or other such entity, to procure background information (also known as a “consumer report and/or investigative consumer report”) about me. This report may include my driving history, including any traffic citations; a social security number verification; present and former addresses; criminal and civil history/records; and the state sex offender records.

I understand that I am entitled to a complete copy of any background information report of which I am the subject upon my written request to theBridge, if such is made within a reasonable time from the date it was produced. I also understand that I may receive a written summary of my rights under the Fair Credit Reporting Act.
